New Patient Registration
Please call in at either site and ask for registration forms at the reception desk. A registration appointment will be required with a doctor and nurse who will carry out registration health checks.
The practice has adequate access and facilities for disabled people. Overseas patients will be required to bring evidence of UK entry clearance for permanent registration.
The practice accepts patients for registration in the postal code areas (DN5/8 DN5/9 DN5/0). Patients have a choice to attend either site. Patients may register as an out of boundary patient by approval of either GP partner in the circumstances of working and visiting in the area (this will not include home visits, where a patient home address falls outside the postal code catchment area).
